I can’t really tell from this angle, but you may need Hayes adapter P/N 98-15073. That replaces the bracket on the caliper, and gives you 51mm IS bolt holes, which should line up to the A2Z adapter.
Another potential (and more expensive) solution would be to get a 51 mm IS mount caliper, and do a new brake.
